A Case of Acute Basophilic Leukemia

Abstract
Acute basophilic leukemia is the rarest leukemia showing prominent basophilia with mature or immature forms at the time of diagnosis. It has rapid clinical course, hyperhistaminemia and poor prognosis. We experienced a case of acute basophilic
leukemia
without preceding chronic myelogenous leukemia. It was diagnosed in a 16-month-old female child on the basis of blasts with basophilic granules in the peripheral blood and bone marrow. She manifested clinical signs and symptoms such as lower
extremity
weakness, pale appearance, ecchymosis, and mild hepatosplenomegaly. The peripheral blood showed anemia, leukocytosis with 58% blasts, and thrombocytopenia. Bone marrow aspiration revealed hypercellularity with 61% blasts of which 50% contained
basophilic granules. Cytochemically, the granules failed to stain for Sudan black B and ¥á-naphthyl acetate esterase, but showed positive reaction to periodic acid Schiff and stained metachromatically with toluidine blue.
